If you are requiring a repeat prescription for your pet please complete the form below in the first instance. We are currently requesting three working days notice. Please be aware of the revised opening days at Cleckheaton and Rothwell, which may impact ordering and collection dates.
You will be contacted once your prescription is ready to collect. If you have not heard from us within 3 working days please call us before visiting the practice to collect. For postal orders please call the Morley hospital.
Orders placed on a Friday after 2pm may not be processed until the following Tuesday and may not be ready for collection / posted until Wednesday.
